Please review your donor selection(s) below before submitting an Order request.

To complete your Order request, please indicate your following preferences: 

• If your clinic has a preference for donor vial type: IUI (prewashed) donor sperm vials or ICI (unwashed). Or, you can select the same donor twice and select each vial type to access a larger inventory of vials.
• The number of vials needed. If you are purchasing vials for immediate use and additional vials to store for future use, please make a note on the order form.
• You can submit just one donor choice, or you may submit up to 3 donor choices, in order of preference (1st choice, 2nd choice, etc). If your first choice is not available, we will attempt to fill your order using your second choice (or third choice, etc). You will NOT be billed for more than one donor per order request.

 

PLEASE NOTE:

We require FOUR(4) BUSINESS DAYS to process completed orders. Orders are not considered complete until we have received: the Order, Payment and Informed Consent (if required).

One or more sperm banks may use the same donor number. Please verify that you have selected the correct donor bank before placing your order.

Please be advised that we have experienced some delays in shipping services due to the ongoing COVID-19 pandemic.  Courier services are no longer guaranteeing overnight service and delays of several days are occurring.  Additional delays due to weather are possible.   Please expect, and plan for, delays when placing your order.
