We understand the importance of protecting your personal information and we are committed to using and disclosing this information responsibly.

In this policy, personal information means information about you that is personally identifiable like your name, address, e-mail address or phone number.

HOW YOUR  PERSONAL INFORMATION IS AVAILABLE TO US

1. By forwarding a completed Donor Sperm Order form.
2. By completed Donor Semen Order form forwarded to us through your physician / clinic for order placement.
3. By providing it to us in communications regarding service delivery such as comment cards and e-mail.
4. By providing the information to us while placing an order for donor sperm or inquiry by telephone or e-mail.
5. By your physician’s office providing Sperm Washing request requisitions.

WHY WE COLLECT PERSONAL INFORMATION

1. To establish and maintain a responsible commercial relationship with you and to provide ongoing service.
2. To identify and to ensure continuous high quality service
3. To enable us to contact you.
4. To establish and maintain communication with you
5. To communicate with fertility clinic/physician.
6. To allow us to maintain communication and contact with you to book and confirm sperm washing and sperm banking appointments.
7. To allow us to efficiently follow-up outcome of treatment and billing
8. For teaching and demonstrating purposes on an anonymous basis
9. To permit potential purchasers, or advisors to evaluate the business
10. To invoice for goods and services
11. To process credit card payments
12. To collect unpaid accounts
13. To assist this office to comply with all regulatory requirements
14. To comply generally with the law
15. To generate billing statements for services rendered or products distributed.
16. To complete and ship orders for donor semen.
17. To comply with legal and regulatory requirements. The Health Canada Semen Regulations require that we maintain a linkage between clients and donor semen samples distributed.

If you do have a concern and/or wish to make a complaint to us about our privacy practices, including asking questions about the contents of your records, you must make your request in writing. Please send it to our office’s Privacy Information Officer by surface mail or fax.

Our Privacy Information Officer will promptly acknowledge receipt of your complaint in writing, and will ensure that it is investigated thoroughly. You will be provided with a formal decision in writing, and the reason for the decision.
